Who provides which services

• Therapy is provided by individual licensed therapists under their own licenses, including Tiffany D. Irving, LMFT (California LMFT #111106 · Nevada MFT #4011-R). Health information you share as a therapy client is protected by federal and state law and is handled as described in our Notice of Privacy Practices. If that Notice and this Policy differ on therapy information, the Notice controls.
• Coaching, groups, and educational products are offered by Diverse Horizons Life Solutions LLC. Coaching is not therapy.

Information we collect

Information you give us, for example when you fill out the contact form, email, call, or book a consultation:

• Name, email address, and phone number
• Your state of residence and current location (so we can confirm a therapist is licensed where you are)
• The service you’re interested in and any message you choose to send

Please don’t include detailed health information in the contact form or in email. We’ll discuss your needs during your free consultation.

You may ask us to tell you what personal information we have about you, correct it, or delete it, and you may opt out of any future sale of your information (we do not sell it). To make a request, contact us using the details below. We’ll verify your identity and respond within 60 days. The Website and our services are intended for adults. We do not knowingly collect information from children under 13.
